Patient Forms

Upon entering our MaxHealth Center office, we will request that you fill out our New Patient forms as thoroughly as possible. For your convenience you can download MaxHealth Center's new patient forms in advance. We also ask that you bring with you a valid driver's license and current insurance card. In cases of car accidents, please bring your current auto insurance. This paperwork provides us with general information about yourself and your current condition.

Private Consultation

An initial "Private Consultation" with one of the doctors at MaxHealth Center will take place on your first visit to discuss your health concerns and goals. This is an opportunity for questions about the nature of your healthcare problem.  The MaxHealth Center doctor will also learn more about your expectations and properly determine if MaxHealth Center can meet those goals.

Physical Examination

A physical examination will be performed by MaxHealth Center doctors utilizing chiropractic, orthopedic, and neurological tests to assess the nature of your healthcare condition.

X-Ray Studies

Your specific condition may require MaxHealth Center doctors to take x-rays for a detailed evaluation. By utilizing x-rays, MaxHealth Center doctors are able to rule out certain conditions and develop more effective treatment plans for you.

Report of Findings

Your general information is combined with your consultation, physical examination, and any x-ray studies to deliver a full report of findings. This answers 4 common questions:

• What is the underlying "cause" of the problem?
• What are the best and safest treatment recommendations to correct the problem?
• What is my insurance going to cover and how much will it cost me out of pocket?
• How long should it take to correct the problem?
